Extreme Makeover Caledonia Edition Official Rules

The Extreme Makeover Caledonia Edition prize includes:

*A fitness program and nutritional counseling for six    months
  (three times a week, schedule to be worked out    between winner and Endurance gym)
*A free dental consultation at Glen Valley Dentistry,   including treatment needed and completed
*A salon appointment at Contempo Salon,including   hair and facial consultation and the services   recommended at no charge
*Nail Care appointment at Main Attraction Salon
*Discounted childcare at The Nanny of Caledonia

*Makeover conclusion package with items from:  Cherry Valley   Greenhouse, RND Health and More, Jamaican Sun, Caledonia   Vision Center, Urban Threads and Essential Bean

To participate in Extreme Makeover Caledonia Edition, the contestant or nominee, must be 18 years or older and fill out an official entry form, in its entirety. (In the event someone is being nominated, the form and requirements will be filled out by the person nominating. This information must be received at/to Glen Valley Dentistry, 9041 North Rodgers, Caledonia MI 49316; or emailed to chris_g@gvdentistry.com by March 31, 2010.

The first place nominator and/or nominee will be notified April 16, 2010, by phone and/or email. If after 3 attempts to contact the winner/nominator have been unsuccessful, it is Glen Valley’s right to proceed to the second place contestant to award the makeover. This process will continue until a contestant has accepted the award and agreed to the participation contract (sample contract available at area businesses)

In the event the winner is contacted and does not agree to the participation contract, the proceeding contestants will be contacted and awarded the makeover and offered the same opportunity to accept the contract of participation.

The contestant winner will be announced in the local newspaper and Glen Valley Dentistry website and must agree to have their photo and name released to the newspaper,website and area postings.

Glen Valley and its participating businesses will not be liable for results during the duration and completion of the makeover. The participant must agree to the result varying depending on the individual and their participation and there will be no guarantees connected with the services provided beyond the six (6) months.

Contest is open to anyone who is able to agree to the contract, but excludes anyone associated with the participating businesses by way of individual employment.

Approximate value of the makeover is $6,000.00

There will be no prize substitutions, assignments or transfers.
